Job Description:

Mission:The Operations Manager (OM) is responsible for providing professional and quality person centered care for residents with Alzheimer?s disease, other types of Dementia, and Non-Dementia care to promote their highest well-being through guiding the Executive Directors. The OM is also responsible for ensuring compliance to company policies and state regulations. This position will function as part of an interdisciplinary team and be able to work independently. The Operations Manager supports and reports to the Director of Operations and Quality Assurance.Ongoing performance evaluations for this job description are based on adherence to Azura Memory Care?s core values and scores attained from performance of the job duties and responsibilities.Job Duties and ResponsibilitiesAdministrative and Operations Management

  • Review with DOQA and provide guidance on financial management of the operating expenses and revenue, projections and records
    • Ensure implementation of operational policies, procedures and systems in all Azura homes
      • Utilize audit sheets for home visits in coordination with DOQA direction
        • Understand and guide Executive Directors in current state CBRF and State regulations ensuring the home is in compliance with DHS 83
          • Maintain standards and comply with state regulations and licensing
            • Maintain and produce reports as requested
              • Ensure proper training is being conducted in the homes for the team members working the floor
                • Ensure Azura?s labor protocols are being followed in the homes, Ensure the established staffing model and staff-to-resident ratios are maintained and budgets being followed
                  • Assist with pharmacy concerns as needed
                    • Assist with program implementations as needed
                      • Assist in addressing family concerns
                        • Conduct interviews as needed
                          • Tour homes regularly for compliance for regulatory and Azura standards for physical plant cleanliness, maintenance, team appearance and resident appearance.
                            • Weekly planning and findings review with the DOQA
                              • Strategizing in collaboration with the DOQA for assisting and supporting Azura House Leadership in Quality Improvement, regulatory compliance, Azura Standards compliance and any area/system identified
                                • Perform onboarding for Executive Directors as needed and fill in the homes in times of Executive Director vacancies or PTO o Coordinate and perform ongoing trainingProgram and Service Implementation
                                  • Ensure the implementation of person centered care through MOSAIC quality of life program to enhance residents mind, body and spirit
                                    • Provide leadership to the MOSAIC program, delivery of care and services to residents with Alzheimer?s disease and other types of Dementiao Encourage, mentor and empower staff to consistently practice MOSAIC program
                                      • Work with House RNs and Director of Clinical Services to ensure resident medical needs are met, best practice is being carried out by Executive Director and staff
                                        • Ensure the rights of residents and the principles of assisted living are upheld
                                          • While in Azura Homes, ensure daily activity programs are occurring as scheduled and meet the needs of the targeted audience
                                            • Ensure meals in the Azura Homes are as planned on the Standardized menu and supporting documentation such as orders are in house, communicated and in place
                                              • Perform on-call responsibilities as needed
                                                • Help to ensure the homes are following Azura standards in ordering food and supplies, being able to fill in as needed for orderingAssessment and Care Planning (in conjunction with Director of Clinical Services and Director of Operation and Quality Assurance)
                                                  • Train on the assessment process for potential residents
                                                    • Review and ensure admissions and discharges of residents are completed according to admissions criteria
                                                      • Ensure the health and emotional conditions of each resident are properly observed and documented to benefit residents
                                                        • Ensure the health and safety of residents with regular training, drills and equipment reviews are performed within company policies and procedures
                                                          • Assure Executive Directors are completing tasks timely and completely as defined in the Executive Director Tasks document
                                                            • Train others as needed and use the electronic medical record to support the Azura homesAll other duties as may be assigned???????Education:
                                                              • A bachelor or associate degree preferred or a minimum of 3 years of related work experience in assisted living or long-term care
                                                                • Successful completion of the CBRF Administrator?s course or meet the qualifications of a CBRF Administrator per DHS 83Experience:
                                                                  • Demonstrated understanding of older adults and the issues, diseases and needs related to aging.
                                                                    • Demonstrates marketing and networking skills to meet occupancy goals.
                                                                      • Demonstrates management skills. Three years? experience in management position preferred.
                                                                        • Demonstrated written and oral communications skills.
                                                                          • Computer skills including word processing.Physical Demands/Working Conditions:
                                                                            • Travel as required up to 80% of the time
                                                                              • Pushing, pulling, moving, lifting and/or carrying items weighing up to 30 pounds
                                                                                • Pushing, pulling, moving and/or lifting a minimum of 30 pounds to a minimum of five feet
                                                                                  • Sitting, standing or walking for extended periods of time
                                                                                    • Ability to read, write, speak and understand the English language
                                                                                      • Talks and listens frequently
                                                                                        • Seeing and hearing or use prosthetics that will enable these senses to function adequately to ensure the requirements of this position can be fully metExperience:
                                                                                          • Demonstrated understanding of older adults and the issues, diseases and needs related to aging.
                                                                                          • Demonstrates marketing and networking skills to meet occupancy goals.
                                                                                          • Demonstrates management skills. Three years' experience in management position preferred.
                                                                                          • Demonstrated written and oral communications skills.
                                                                                          • Computer skills including word processing
                                                                                          • Long term care experience requiredPhysical Demands/Working Conditions:
                                                                                            • Travel as required up to 75% of the time
                                                                                            • Pushing, pulling, moving, lifting and/or carrying items weighing up to 30 pounds
