If so, iCare is looking for a Nurse Practitioner join our growing team in the Family Care Partnership (FCP) Program! The NPis responsible for providing advanced level case management and care coordination for Family Care Partnership members in collaboration with the Interdisciplinary Team (IDT), the IDT Supervisor. The NP also provides clinical and administrative support for initiatives to achieve member health outcomes related to program and company objectives.ESSENTIAL DUTIES and RESPONSIBILITIES:Member of the IDTUnderstand the Family Care model and work within these parametersWork collaboratively with the IDT Staff and Supervisor to provide clinical expertiseProvide assessment for new members as needed and members experiencing acute care needs and collaborate with the Supervisor and IDT on the findings and plan developmentDiscern member needs that require NP level intervention, work with Supervisor and IDT to create a plan to address themProvide assessment and intervention for identified members from the Supervisor and IDT, collaborate and communicate on findings and plan developmentEnsure appropriate care management for members identified as needing NP involvement.Provide clinical expertise as needed in routine team meetings and case staffingProgram Support RolesAccept assignment to special areas of focus for program and companywide initiativesProvide clinical expertise to leadership in the development of strategies to achieve member health and company objectivesDevelop protocols documenting strategiesCollaborate in the development of metrics to evaluate successiCare Perks:Full Time associates enjoy:Monday - Friday, no evenings/weekends/holidaysMedical, Dental, Vision, and a variety of other supplemental insurancesGenerous PTO and paid holidays401(k) Retirement Savings Plan with Company MatchTuition ReimbursementAnd much more!QUALIFICATIONS:Master's Degree in Nursing.Possession of a licensure or eligibility for licensure in the State of Wisconsin as a Registered Nurse, Advance Practice Nurse Prescriber (APNP) licensure, and national NP certification in geriatrics, adult or family practice.Strong assessment and care planning skills required along with the ability to provide complex care coordination.Ability to effectively communicate thoughts, ideas, and information both orally and in writing.Ability to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects.Strong interpersonal skills and ability to effectively interact with providers, members and co-workers from a variety of different backgrounds and experiences in a professional and courteous manner.Ability to work effectively as a team member and cooperate in achieving company goals.Ability to think creatively and solve complex problems using best practice standards within the context of a managed care environment.Must be able to travel to any location in Dane and Sauk counties with some travel possible in other counties within Southeast WI.A personal vehicle, valid State of Wisconsin motor vehicle operator's license and conformity with insurance coverage limits are required.Associates are required to be fully COVID vaccinated or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved vendor and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into an iCare facility or work outside of their home.PREFERRED QUALIFICATIONS:Previous home health, managed care or case management experience.Previous experience working with persons in any of all of the target member populations: (frail elderly, physically disabled, developmentally disabled) in a health care setting.General knowledge of Medicaid and Medicare benefits.
