We have 18 residential homes throughout Southeastern Wisconsin, each with 2-4 residents that are ambulatory, active in the community, and uniquely engaging to learn and grow with. We are looking to add a dedicated, timely, and professional Licensed Practical Nurse to our Management Team to provide world class support to our residents and staff!Location: central office in Port Washington, WI, with some travel to our homes as neededSchedule: full-time, 40 hours per work weekWage: $25 per hourDuties:Observes and reports changes in client condition, or any other concerns and transitions, to the physician, family, and case manager in a timely and professional manor.Generates and reviews Individual Service Plans (ISPs) and consults about client conditions and their treatments.Assists with implementation of medical orders.Carries out proper charting practices and procedures, and provides additional guidance as needed to existing staff in noting and charting.Attends rotating house meetings, and any necessary in-service training or conferences as needed.Consults with the Director of Operations regarding staff performance problems and successes.Requirements:Possess a valid, active LPN license in the state of Wisconsin, and maintain current licensure.1-3 years experience in the healthcare field.Ability to pass a background check.Possess a valid Wisconsin Driver's License with ability to travel to all of our worksites.Ability to stand, bend, and walk and possess full ambulation.
