The Admission Coordinator is an internal customer service position responsible for coordinating the referral-to-admission process for home health, hospice, and palliative care patients. They provide administrative support and ensure that all eligible patients receive care, barriers to admissions are addressed and resolved and agency(ies) growth and profitability is maximized. We have a 10:30am-7pm and 8am-4:30pm Monday - Friday shift available. Limited after hours on call required. On call is done remotely but weekday hours are located in our Milwaukee office off of Innovation Drive. Educational Requirements

Associate Degree in related field required. Requisite experience may be considered in lieu of Associate Degree. Bachelor's degree preferred.

Position Requirements

Demonstrated customer service experience Health care experience required. Travel is minimal but necessary on occasion as directed by sales manager.
