The LPN provides advanced wound care, durable medical equipment, and other services to patients who are in various care facilities. By leveraging the power of Artificial Intelligence, we have been able to heal wounds faster and more efficiently. The LPN will be a part of our companys mission to heal wounds and prevent amputations in various care facilities.REPORTS TOThe LPN reports to the following:Chief Executive OfficeAssigned Supervising Physician per StateNational Medical DirectorDirector of Clinical Services and Assistant Director of Clinical ServicesSenior Vice President of Durable Medical EquipmentAssigned Clinical SupervisorAssigned Regional Team Lead (if applicable)Assigned Supervising Nurse Practitioner/Physician Assistant/Doctor of Podiatric Medicine/Doctor of MedicineAssigned Registered Nurse (if applicable)RESPONSIBILITIES AND DUTIESAbide to the clinical departments policies and proceduresPerform complete physical exams of patientsConduct initial and follow-up visits, collaborating with the assigned supervising Nurse Practitioner/Physician Assistant via Telehealth as neededRecommend and perform ordered tests and procedures.Perform wound care procedures as deemed necessary and appropriate for a plan of care in accordance with Wound Pros 7 Step Treatment ProtocolPerform a comprehensive plan of care for documented patient diagnosesRequest for Durable Medical Equipment per companys guidelines and protocolsRecommendations for prescription of required medications and/or Surgical/Wound Care Dressings to facilitate wound healing and/or wound preventionAssure all patients are completely satisfied, by exemplifying Wound Pros standards for patient carePatient education and managementAssists with special projects as neededConservative debridement of wounds during the patient visitApplication of topical wound care and other wound management productsNotate findings and treatment course in-patient chart, including follow-up notesActively participate in departmental activities to improve training methods and achieve company goalsTravel to patients homes and/or long-term care facilities will be required, a 50-mile travel radius from the clinicians home is expectedTravel assignments to new market areas per companys needs and growthMeet the documentation requirements of Medical Records DepartmentMaintain excellent communication with all stakeholders, agencies, patients, and familiesBecome an expert at wound care consultant treating our patients chronic non-healing wounds, delivering excellent customer experiencesCrush quarterly goals through consistency in providing tremendous care, administering biologic application where necessary for rapid healing ratesSeek personal and leadership growth through participation in departmental activitiesMeet metrics designed for field clinicians, including maximizing patient visits, both telehealth and face-to-face, utilizing treatment modalities to demonstrate efficient wound healing and minimize time-to-healing ratePROFESSIONAL QUALIFICATIONSLPN must be a graduate of an approved LPN ProgramCurrent active license to practice as a LPN in the StateMust have prior Wound Care experience and/or Wound Care CertificationFull-time experience working in a skilled nursing facility is a plusValid and unrestricted drivers licenseCurrent BLS certificationCurrent malpractice insurancePERSONAL CHARACTER QUALIFICATIONSDemonstrate the Wound Pros core values: We Listen, We Innovate, We Never Give Up, We Kultivate and Grow PeopleExcellent communication skillsExperience working with healthcare related technologies, data, and software(s)Prior experience in cross-functional roles and team collaborationStrong analytical and problem-solving skillsExcellent organizational skills, ability to multitask, and time managementTechnical aptitude with ability to adapt, learn quickly, and teach othersProficient use of MS Office Suite (particularly Excel and PowerPoint), Google Sheets, Google Docs, Google Drive etc.JOB REQUIREMENTMust be wiling and able to travel within the state or out of state for training.Patient-centric approachExcellent verbal and written communication skills in a professional environmentMust be proficient with medical instruments and equipment required by Wound ProsAbility to communicate effectively, in verbal and written formSound understanding of all federal and state regulationsAbility to lift 50 lbs. or moreMust have a full range of mobilityMust be able to reposition patientsCOMPENSATIONTotal compensation potential of up to $79,000.00 per year, plus tremendous opportunity for professional growth and advancement.WORK LOCATIONAs a mobile Clinical Provider, you will be required to travel and treat our patients who are homebound or in long-term care facilities.BENEFITSHighly competitive compensation packageYou will have the full support of Wound Care Certified Nurses for consultationPTO days awarded after 30 days of employment.Sick Day & Bereavement DaysMedical, Dental, and Vision insurances401K plan availableMileage ReimbursementCALL TO ACTIONWe are looking for superstar A-Player to join our growing team!
