Nursing Care Specialist Role
Ascension Wisconsin is seeking a dedicated Nursing Care Specialist to join our team of healthcare professionals. As a key member of our nursing staff, you will provide direct patient care and coordinate with other disciplines to ensure well-coordinated care.Responsibilities

• Implement and monitor patient care plans to ensure optimal outcomes
• Monitor and record patient condition, communicating effectively with the healthcare team
• Coordinate patient discharge planning, working closely with other disciplines to ensure a smooth transition
• Note and carry out physician and nursing orders, prioritizing patient care
• Assess patient needs and develop individualized care plans to meet those needsRequirements
  • Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or current home state license for multi-state license recognition
  • BLS Provider credentialed from the American Heart Association
  • Graduate of a board-approved Registered Nurse Program
  • RN with approximately 18 months or more experience preferredBenefits
