Day.Aspirus Wausau Hospital in Wausau, WI is seeking a SOCIAL WORKER to join our CARE COORDINATION team!*** SIGN ON BONUS ELIGIBLE!!The Social Worker is responsible for assessing patient needs and coordinating referrals to resources to meet those needs. As part of the multidisciplinary team, the Social Worker works closely with physicians, nursing, and ancillary departments to effectively coordinate the patients plan of care. The Social Worker will attempt to resolve social, economic, and environmental difficulties interfering with the patients ability to manage their health needs effectively.
