Shifts per Week: 3 (blocked schedule: Friday, Saturday, Sunday;
12-hour day shifts, 36 hours per week) - Estimated Weekly Pay:
$1,935 - $2,150 - Employment Type: Travel Nursing Contract
(On-Site) Registered Nurse Qualifications - Minimum 12 months of
recent Home Health RN experience (within the last 3 years) -
Proficiency with HCHB (Homecare Homebase) – required - OASIS
documentation experience – required - Active Wisconsin or Compact
Registered Nurse License – required (must be in hand at time of
submission) - Current BLS certification – required Home Health RN
Responsibilities - Provide skilled nursing care to patients in
their homes across the Greater Milwaukee area - Complete three
patient admissions per day and approximately two additional home
visits daily - Utilize HCHB and OASIS for accurate, timely clinical
documentation - Collaborate with interdisciplinary healthcare teams
to develop and implement individualized care plans - Educate
patients and families on disease management, medications, and
self-care techniques - Ensure compliance with all regulatory,
