Job Description Note – This is a Temp to Hire role Position – Patient Care Navigation Spec Location – Milwaukee, WI Contract – 12 Months with a possibility of extension Job Summary Coordinate patient navigation and care access across multiple medical specialties. Provide high-touch, white-glove service to diverse and high-profile patient populations. Manage intake, triage, and care coordination to ensure seamless patient experiences. Collect and maintain comprehensive patient data to support personalized care delivery. Key Responsibilities Coordinate patient intake and triage across multiple service lines. Navigate patients through the healthcare system and connect them with appropriate specialists. Manage complex scheduling and follow-up care, including labs, imaging, and treatments. Serve as a liaison between patients, families, providers, and leadership. Deliver exceptional patient advocacy, service recovery, and care coordination. Maintain accurate documentation using EMR and CRM systems. Collaborate with department leadership on multi-specialty and complex care plans. Required Skills & Experience 4 years of experience in intake, scheduling, referrals, or care coordination across multiple service lines. Strong logistics, organization, and multitasking abilities. Excellent communication, patient advocacy, and customer service skills. Proficient in Microsoft Office; knowledgeable in medical terminology. Ability to work independently with strong follow-through. Preferred Qualifications Experience with Epic EMR. Bachelor’s degree preferred (Associate degree or equivalent experience required).
