Job Description Care Navigation Specialist - Hybrid Pride Health is hiring a Care Navigation Specialist for one of its clients in Milwaukee, WI. This is a 12-month contract with a Temp-to-Hire possibility, with competitive pay and benefits. This is a hybrid role. Candidates will need to be local and okay with coming in, depending on the needs of the department. Location – Milwaukee, WI 53226. Length of assignment - 12 months (Possibility of Temp-to-Hire) Pay range - $25 - $28 per hour. Shift/Schedule: M-F 8 am - 4:30 pm. Job Summary Coordinates patient navigation through the healthcare system, effectively communicating with patients, families, and providers to identify needs and connect them with appropriate resources. Manages interactions with high-profile patient types, including community leaders, political figures, philanthropists, board of trustees, and celebrities, demonstrating exceptional interpersonal skills and providing "white-glove" service. Manages the intake, triage, and coordination of care for patients across various specialties and service lines, including Neurology, Cardiology, OB/GYN, Primary Care, Oncology, Orthopedics, and Dermatology. Ensures comprehensive data collection on each patient, including medical history, financial status, and relevant records, to facilitate personalized care. Job Duties Patient Intake and Triage: Coordinate the intake and triage process for patients across multiple medical specialties/service lines, ensuring accurate collection of medical history and relevant information. Care Coordination: Navigate patients through the healthcare system and facilitate connections to appropriate specialists based on their needs. Multidisciplinary Follow-up: Manage complex follow-up care logistics and coordinate appointments for new/established patients, including labs, scans, and treatments. Exceptional Patient Service: Provide high-touch patient service, maintaining exceptional communication skills and fostering a positive experience for patients and families. Data Management: Utilize electronic medical record (EMR) systems and customer relationship management (CRM) systems effectively to maintain patient records, manage care transitions, and gather necessary medical documents. Communication: Act as a liaison between patients and healthcare providers, communicating appointment details and treatment plans effectively. Work with department leadership to coordinate patient appointments across multiple service lines or with complex care plans. Service Recovery : Address patient concerns and restore trust by providing exceptional service recovery and advocacy when issues arise. Requirements Associate degree required. instead of a degree, an additional two (2) years of experience in medical intake, scheduling, referral facilitation, or related duties. Bachelor's degree preferred. Minimum of four (4) years related experience (intake, scheduling, referral facilitation, care coordination) across two (2) or more service lines. Needs to have very good hospitality skills, be able to multitask, and be able to problem-solve. Logistics management capabilities. Patient advocacy and service recovery expertise. Teamwork and collaboration skills. Excellent computer skills; proficiency in Microsoft Office Suite.
