community! Responsibilities About the Role During visits that may
last up to one hour, our providers assess and evaluate a member’s
current health status, medical history, medication compliance,
social circumstances, and other potential risks. This approach
provides invaluable insights into a member’s overall health that
are often missed during standard office visits. The Matrix
Comprehensive Health Assessment (CHA) aims to enhance care quality
and allows us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, which include reviewing
medical history, diagnosing and treating conditions, providing
health education, making physician referrals, and ensuring thorough
documentation in line with our guidelines. Administering and
ordering necessary point-of-care and lab tests. Collaborating with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as needed. Working
alongside Primary Care Physicians (PCPs) to educate patients and
follow up as required. Delivering services at the member’s
residence, which may include their homes or Skilled Nursing
Facilities. Benefits of Working with Us Consistency – While
assessments are conducted from Sunday to Saturday between 8 am and
7 pm, our full-time providers generally work Monday to Friday from
8:00 am to 5:30 pm. Competitive Pay – Enjoy an annual base salary
ranging from $105,500 to $168,400 , determined by market analysis
and experience. Incentive Program – Benefit from uncapped quarterly

onboarding training, you will be paired with a preceptor, receive
personalized feedback, and gain free access to online continuing
education. Additional Licensure – If business needs arise, we will
assist you in obtaining additional state licensure and
credentialing in neighboring states or others you may wish to visit
while conducting health assessments. Qualifications Skills &
Experience That Shine A Master’s Degree or equivalent experience
along with successful completion of NP or PA licensure. Current
NP/PA licensure in the state of practice, including prescription
authority or the ability to obtain it as necessary. Board
certification from NCCPA, AANP, AACN, or ANCC in Adult, Adult
Gerontology Primary Care, Adult Gerontology Acute Care, Family,
Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR
certification. A minimum of one year of NP or PA experience.
Willingness to travel up to 80-90% of the time, including overnight
trips and weekends across the assigned state and possibly
neighboring states. A valid state driver’s license and proof of
adequate automobile insurance for your state of residence. Strong
computer skills and familiarity with health/medical record
software. Excellent verbal and written communication abilities. The
