Home Health & Therapy! We are looking for a licensed Physical
Therapist to join our compassionate and growing team in the Green
Bay area. This is a great opportunity to build meaningful

in your schedule. As a clinician with PRN HH&T, youll primarily
work in a home health setting, with additional opportunities for
cross-training and work in skilled nursing facilities if desired.

interaction Job Responsibilities: Perform in-home patient
evaluations and develop individualized treatment plans Deliver
evidence-based therapy to improve patient mobility, function, and
quality of life Collaborate with patients, families, and clinical
team members to support care goals Accurately document treatments
and progress, including discharge summaries Supervise and guide
Physical Therapist Assistants (PTAs) in treatment plans Educate
patients and caregivers on safe mobility practices and discharge
planning Requirements: Current Physical Therapist (PT) license in
the state of Wisconsin Valid drivers license and reliable
transportation 1-3 years of Home Health experience preferred but
