Job Description Up to $47,000/year starting salary plus $1,000 upon
certification We are a dynamic team that provides Wraparound/REACH
services to members enrolled in the Wraparound Milwaukee program
here in the greater Milwaukee area. JFS Care Coordinators assess,
monitor and implement strength-based treatment plans for child(ren)
involved in the juvenile justice or child welfare system while
creating the least restrictive living arrangements for these youth.
We are looking for someone with a 4-year Bachelor's or a Master's
degree in a Human Services field (Social Welfare, Psychology,
Social Work, etc.) AND have at least one-year of paid, formal
experience providing case management services. If you have the
ability to work independently, stay organized, are
detailed-oriented, able to meet deadlines (enter timely case notes,
submit reports, etc.), and most importantly, conduct assessments
of, monitor and support Wraparound consumers with a professional
and caring methodology, we would like to meet you. Our Care
Coordinators meet with individuals in their homes or out in the
community to identify and utilize local community resources for
client families and children while coordinating with intra- and
inter- agency resources to provide the delivery of services and

position is full-time, predominantly first shift position requiring
a valid Wisconsin Driver's License, access to a vehicle, personal
motor vehicle insurance meeting the minimum requirements of the
Agency, passing of a Department of Justice Background Check and
Federal Sex Offender Registry Check, and verification of
references. Duties and ResponsibilitiesDevelop child and family
teams.Advocates for services for the child & family.Assess needs of
children and families.Secure placement for children as
needed.Identify and utilize local community resources for client
families and children, coordinate with intra- agency and
inter-agency resources the delivery of services and treatment
planning.Provide written documentation to include progress notes,
the completion of formal written treatment summaries,
recommendations, treatment plans, contracts, and other required
reports.Represent child's best interest in court while representing
agency at court, school, medical and other community
resources.Prepare all documentation according to time frames and
program guidelines.Ability to conduct and coordinate individual,
family and group therapy sessions utilizing home, community and
institution-based resources and techniques, strategies and
interventions for delivery of home-based family services.Ability to
prepare formal reports.Ability to display cultural competence by
responding respectfully and effectively to people of all cultures,
languages, classes, races, ethnic backgrounds, religions and other
diversity factors in a manner that recognizes, affirms and values
the worth of individuals, families and communities and protects and
preserves the dignity of each. GeneralMust have a valid driver's
license, access to reliable vehicle, and automobile insurance
coverage throughout employment and successfully pass a MVR and
insurability check.Knowledge, understanding, and/or willingness to
learn Jewish culture and traditions and those of the Agency
population as necessary.Accept other duties and responsibilities as
assigned.Candidates must successfully pass a criminal background
check with the Wisconsin Department of Justice and a caregiver
background check with the Wisconsin Department of Health Services,
the Department of Children and Families and the Department Safety
and Professional Services.Candidates must pass a Dru Sjodin
National Sex Offender and Wisconsin Sex Offender Registry
screening.Documentation, or verified medical exemption, of
immunizations and screening: Tuberculosis, Flu, Measles, Mumps,
Rubella, and Varicella (Chicken Pox). Four-year Bachelor's degree
in Human Services (to include, but not limited to Social Work,
Psychology, Sociology, Criminal Justice, Mental Health Services,
AODA Services) is required. Master's Degree preferred.A minimum of
one (1) year of paid formal experience working in Case
Management.Knowledge base of adolescent and family issues and
dynamics.Knowledge of the Wisconsin Children's Code and juvenile
justice systems.Knowledge of local, state and federal statutes and
regulations, guidelines, practices and policies relevant to or
specifically applicable to their specific role and the operation of
youth programs, out of home placement of children, abuse and
neglect reporting procedures and requirements, termination of
parental rights and children in need of protective services and the
educational system.Knowledge of Milwaukee County (Department of
Human Services, Children's Court, Milwaukee Public Schools, and
Milwaukee community-based youth serving agencies.)Knowledge of
