Position Description The Bereavement Coordinator ensures the overall administration and implementation of bereavement services for the Hospice Care Program provided before and after the death of the client while adhering to Medicare and state-specific regulations. The Bereavement Coordinator will provide assistance with issues related to grief and loss, as well as spiritual support and any other counseling services for the client and family while the client is enrolled in the Hospice Care Program. Essential Duties and responsibilities * Provides bereavement services after the clients death and makes contact with patients, caregivers and/or family members in need of bereavement support prior to death, when referred * Provides spiritual support and any other counseling services for the client and family while the client is enrolled in the hospice program * Coordinates the assessment and delivery of bereavement and grief counseling needs and services (via phone, office visits, home visits, groups and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers * Provides consultation, education, outreach, bereavement information, and referral to Hospice team members, other professionals, and the community at large * Expands and maintains community bereavement resource list as needed * Assists in the ongoing development and implementation of an organized program for the provision of bereavement services including but not limited to events such as Hospice Memorial Services, Grief Support Groups, and Bereavement Care Seminars * Makes bereavement services available to the family and other individuals in the bereavement plan of care up to 1 year following the death of the patient. Bereavement counseling also extends to residents of a SNF/NF or ICF/IID when appropriate and identified in the bereavement plan of care * Develops a bereavement plan of care that notes the kind of bereavement services to be offered and the frequency of service delivery * Participates in interdisciplinary group meetings and actively communicates with the nursing supervisor and other team disciplines * Maintains strict patient confidentiality * Collaborate with other interdisciplinary team members to promote ease of transition from patient care to bereavement services * Attends all mandatory in-services and meetings provided by agency * Performs additional duties as assigned REQUIRED Knowledge, Skills and Experience * Bachelors degree from an accredited school in social work, psychology, counseling, pastoral education, or related field * Current drivers license and automobile in good working condition with proof of auto insurance * Documented evidence of training and experience in dealing with grief and loss counseling * Be available at all times during operating hours and as bereavement needs arise * Has the mental and physical health necessary to meet the demands of the position * Has excellent written and verbal communication skills * Have satisfactorily passed Agency pre-employment testing and field competencySDL2017
