Westminster Place - LPN - Full Time - (11pm-7am) – Every Other Weekend (Saturday, Sunday) - Skilled Nursing Department. Complete admission records and initiate personalized care plans. Respond promptly to urgent and emergent situations. Maintain a safe and supportive environment for residents. Implement and monitor individualized care plans. Supervise and mentor nursing assistants, ensuring high-quality resident care. Document essential information accurately in medical records. Monitor residents' health status and communicate effectively with supervisors. Build and maintain therapeutic relationships with residents. Participate in care conferences and ongoing staff development programs. Utilize computer software for efficient documentation and communication. Complete required computer education modules. Adhere to corporate compliance policies, ensuring confidentiality and resident well-being. Administers medications and treatments in accordance with established policies and procedures. Is familiar with indication for use, action, and major side effects of medications administered. Education: Graduation from an approved registered professional nursing program. Qualifications: Active, current Illinois license to practice professional nursing. CPR Certification Required - Accredited by American Heart Association. One year as an RN in a hospital (general care) or long-term care setting with adults preferred. Genuine interest in working with older adults. Knowledge of aging changes and common health problems of geriatric patients. Good communication skills. Ability to direct nursing assistants.
